Lancaster-Lebanon Intermediate Unit 13 has partnered with Walmart Pharmacy to host a COVID-19 vaccination clinic for children age 12 and older from all public school districts in Lancaster County, including Penn Manor.
The Pfizer vaccine will be administered from 3 p.m. to 8:30 p.m. Thursday, May 20, and Saturday, May 22, at IU 13 headquarters, 1020 New Holland Ave., Lancaster 17601.
A second dose of the Pfizer vaccine will be administered at the IU headquarters from 3 p.m. to 8:30 p.m. Thursday, June 10, and Saturday, June 12, to complete the vaccination series.
Participation is voluntary for families who would like to have their child/children vaccinated. The vaccines are being offered to families at no out-of-pocket cost.
To receive a vaccine, parents/guardians must schedule an appointment ahead of time by clicking this link or this link. Parents/guardians also can use the QR code below to reach the registration site on their cellphones.
They also must complete these three patient forms: the Walmart Vaccine Administration Record, Insurance Attestation and Minor Consent Form. The forms also are available for download on the registration site.
Parents/guardians should bring the completed forms to the clinic, along with health insurance information. Additional forms will be available on site, but vaccines will not be administered to minors without guardians present unless the Minor Consent Form is completed.
